For patients managing their physical health with ADHD, autism, or the chronic conditions that commonly co-occur [POTS, hEDS, HSD, chronic fatigue, dysautonomia and similar], knowing what to do and being able to do it ongoing can be two very different things.
Standard approaches to behaviour change often don't land. Not because patients or clients aren't trying, but because those approaches weren't designed with their reality in mind.

Research shows that multidisciplinary support improves outcomes for patients with chronic conditions¹, and that dedicated self-management support is often what's missing between good clinical advice and lasting behaviour change.
I work alongside GPs, specialists, exercise physiologists, dietitians and other allied health practitioners, through either referral or as embedded support within your practice.
Referral pathway
Patients & clients can self-refer or you can refer directly using the practitioner’s referral form. I conduct my own intake process to ensure we’re a good fit before beginning working together. Provided we are a good fit I’ll work with them independently, with communication back to you as appropriate.
Embedded support
For practices wanting a more integrated approach, I work with select clinics on an ongoing basis to provide consistent support for your patient community. This might look like regular availability aligned with your practice, streamlined referral processes or a closer collaboration on patient care.
